Caring for a loved one with a neurocognitive disorder can be an overwhelming and exhausting journey. At our practice, we recognize the immense physical, emotional, and mental toll this responsibility can take on caregivers. Caregiver fatigue and burnout are real.

Therefore, we offer direct communication with our team. Our goal is to alleviate some of the burdens by offering professional triage, coaching, and support tailored to each family's unique situation.

We aim to empower you with the knowledge, resources, and emotional support needed to navigate this difficult path. Our dedicated team is committed to standing by your side, ensuring that both you and your loved one receive the care and attention you deserve. 

The Core Meeting is an exclusive service offered to members of our membership program. It is a 30-minute meeting directly with their Physician (s). We open this meeting to include caregivers and family to inform everyone caring for the patient.

Our Physician (s) will offer comprehensive guidance on the patient's care plan, education regarding the illness, the disease's natural progression, expectation management, different environmental interventions, and healthcare decision-making. There is a heavy emphasis on coaching families through the steps to ensure the highest quality of care for their loved ones, including care for themselves.

This meeting will occur over Zoom. Therefore the invitation is open to include any preferred person for the patient. Our practice will present information specifically curated based on the patient's symptoms and your family's interests and needs.

Our practice manager will organize the Core Meeting for administrative ease, or the patient's point of contact can set the appointed meeting time directly with our practice manager.

Two core meetings are allowed per membership year. Our Physician (s) time these meetings based on the best clinical care for our patient. These meetings are not only a valuable opportunity to discuss treatments and future care plans but also serve as a platform to get family and caregivers on the same page, fostering a collaborative approach to patient care. 

We invite our patient's point of contact (POC) to attend the follow-up appointments in person. We find the patient is usually more at ease when someone they are familiar with is present. This is not a requirement. If you're unable to be present, depending on the care needs, we offer remote access for families during the patient's appointment. And we work to help arrange, coordinate, and inform our families when they can expect our physicians to round.
